Advantage or Disadvantage on Attack Roll

Here are some reasons why an attack roll would have advantage or disadvantage. Not listed are spell and class feature effects.

Attack Roll has Advantage

Attacker's ally takes the Help action
Attacker and ally flank the enemy
Attacker attacks from the enemy's rear
Target suffers a debilitating condition
Attacker is unseen
Enemy is in a tight space
GM discretion
GM grants attacker inspiration

Attack Roll has Disadvantage

Enemy takes the Dodge action
Enemy is long range
Attacker's ranged attack is too close
Attacker suffers a debilitating condition
Attacker is in a tight space
Enemy is unseen
GM discretion
Weapon is too heavy for attacker
Attacker lacks armor proficiency
